"Healthwise for Life" Program -

     This 16-week, free program is a home-visiting program of preventive healthcare, education, exercise, and networking with community services. An Outreach Specialist comes to your home for an hour each week to check your blood pressure, exercise, and provide information to encourage a better doctor/patient relationship.

Each week the visitor will:

  • Utilizes the Healthwise for Life - A Self-Care Guide for People Age 50 and Better during their visit to provide education for living a healthier, safer life at home. It provides information for first aid and emergencies, common health problems, living better with chronic disease, staying healthy, and explores the myths, gifts, and realities of growing older;
  • Record your blood pressure on a card that you can keep with you to take to your doctor's appointments;
  • Provide a theraband to encourage movement and engage in other appropriate exercise suited to the person's ability;
  • Network with other resources help with unmet needs of the individual; and
  • Be a friend.
  • If desired, the Outreach Specialist will help compile a list of medicines and a health history (to be retained by the participant), assist with completing a Living Will, and do a video interview, recording the person's life story.

     After the initial 16 weeks have passed, our Outreach Specialist will return on a monthly basis for as long as the individual would like.
